I wish to enroll my child in Camp Eagle Ridge.  I am enclosing a non-refundable deposit in the amount of $50.00 per week to be applied toward tuition costs.  I agree to pay the balance of the camp fees by May 15, 2008.  If I do not pay the balance by that date, I risk losing my child’s spot in camp and forfeiting my deposit.  I understand that campers are admitted to Camp Eagle Ridge with the agreement that they shall respect and adhere to the rules and regulations of the camp.  No rebate of tuition or other charges will be refunded in the event that the camper is dismissed or suspended.

I give my permission for the above listed emergency contact to pick up my child from camp if the Camp Director deems that necessary.

I also authorize the use of any photos or video of my child for public relations and promotional purposes.
